We Offer Home Infusion Services to Massachusetts, Rhode Island, and Southern New Hampshire

    For our Patients...

     

    • You will be supported by a dedicated TEAM of experienced infusion nurses who will provide your treatments in the comfort of your home.
    • A member of our Clinical Leadership Team will call you prior to your first appointment to learn about your needs.
    • We provide seamless coordination between your physician, pharmacist, and nursing team to deliver comprehensive infusion care.
    • A skilled Scheduling Team will work closely with your infusion nurse to arrange convenient and timely appointments tailored to your needs.
    • Your nurse will provide personalized education about your infusion therapy, empowering you with the knowledge to manage your care confidently.
    • Our Patient Care Team will assist you with any clinical questions and will assist your nurse in your home as needed. 
    • You will have access to 24-hour nursing coverage to address any urgent needs outside of regular business hours.

    How Scheduling Works

    • T.L. Connections makes every effort to accommodate your appointment requests.
    • You will have a team of nurses assigned to your case.  A team nursing approach ensures that you will have nurses that are familiar with your care.
    • Although we do see patients on the weekends, weekend visits are not guaranteed due to a limited number of staff available on the weekend.
    • At each visit, please speak with the nurse about your next scheduled appointment.
    • We ask that if you need to reschedule your appointment, you contact us two weeks prior to your visit.  Requests for visit changes with less than 2 weeks’ notice cannot be guaranteed.
    • If an emergency arises and you need to cancel or reschedule your appointment, please call us as soon as possible so that we can make every effort to reschedule you.
    • Please do not text for cancellations within 24 hours.  Please call the office and speak with Scheduling or the RN on Call for cancellations within 24 hours.
    • The week prior to your appointment, you will receive notification from the office with your appointment day and time.
    • When you confirm your appointment via text, you will receive a second text asking to confirm that you have medication in your home.
